The following terms and conditions are agreed to and must be met by Client Company.  The signature of the Client or agent constitutes full acceptance of the following effective as of the date of signing and will remain in effect until terminated by UniStaff.   It is agreed that the Client shall accept full responsibility for bodily injury, property damage, fire, theft, collision, or public liability claims arising out of the operation of the Client’s motor vehicle (excluding forklifts), whether owned or rented by a UniStaff employee. Client will not permit any UniStaff employee to operate any motor vehicle (excluding forklifts), climb ladders or scaffolding, lift over 50 lbs. or operate machinery of any kind in the performance of their duties without first obtaining prior written consent from UniStaff. It is agreed that the Client will not entrust a UniStaff employee with any unattended premises or with the care, custody, or control of cash, negotiable, or any other valuables without prior written consent from UniStaff, and then only when the UniStaff employee’s specific duties necessitate such activities. It is agreed that the Client will provide safe places of employment and to establish sound operating practices which will result in safe working conditions for UniStaff employees that comply with all laws and ordinances related to occupational safety and health. Client agrees to accept the responsibility of direct supervision of UniStaff employees on your premises.  Client also agrees to implement safety principles and provide knowledge essential for the development and understanding of proper safety, attitudes and to provide an orientation to each new employee of those procedures.  Client assures that necessary safety equipment and personal protective equipment will be made available and used by UniStaff employees while working on your premises.  Client also agrees to fax a copy of any training or testing materials to UniStaff’s corporate office. It is agreed that the Client shall notify UniStaff of any changes in the duties of a UniStaff employee from those originally described to UniStaff. Client agrees to report any work-related accident or injury to UniStaff within 24 hours.  Client further agrees to cooperate with investigation procedures, including but not limited to, any interviews, forms, and/or on-site inspections. UniStaff has implemented a Modified Duty Return-to-Work Program.  Client agrees to provide UniStaff employees a light duty position, if available, following an accident on client premise if the terms of light duty can be met. All hours worked over 40/week will be deemed overtime and billed at time and one half. Minimum assignment length is 4 hours.  UniStaff complies with IDOL regulations. UniStaff invoices on a weekly basis with payment terms of net 30 days.  Mailing address is:  301 E. North Avenue., Northlake, IL 60164. If a UniStaff employee is assigned to Client Company and shows up at the appointed time, but is not placed in a position or does not work the full four hours, it is mandatory according to IDOL regulations that employee get paid the four hours.  This will then be billed to Client Company. Client agrees to pay collection costs and interest at 1 ½ % per month on delinquent accounts 30 days past agreed upon terms.  It is agreed that arbitration will be used for all disputed invoices. UniStaff complies with all Federal, state and local laws/regulations including, but not limited to:  Social Security Administration, in an effort to fulfill verification laws currently in effect.  UniStaff is also compliant with the recent Employment Day and Temporary Labor Services Act as well as all laws, regulations and rules relating to the services to be performed by temporary personnel.
